Intoducing Microdissection Testicular Sperm Extraction (Micro-TESE)

Micro-TESE is an advanced procedure that requires the skill and understanding of an expert micro-surgeon very familiar with testicular tissue. The combination of using the operating microscope, dissecting the testicular tubules and having trained personnel available to search for sperm can make the difference between success and failure for many couples.

Micro-TESE can be performed as a diagnostic procedure and if usable sperm are found, then they can be frozen and the couple is recommended to proceed with ICSI. It can also be performed and timed with an egg retrieval/IVF cycle so that the sperm are injected into the eggs without freezing. Freezing the sperm from men with sperm production problems can be difficult since these sperm are usually few in number and don't thaw well.

Therefore the best chance of pregnancy is to use fresh sperm obtained just prior to IVF. The chance of finding sperm with Micro-TESE is better than 60%. This is twice the chances of finding sperm by non-microsurgical or needle biopsies taken by general urologists.

Micro-TESE is a great advance in male reproductive surgery, but is only performed by a small number of male reproductive surgeons. Some surgeons make unnecessarily large incisions in the scrotum and the testicle. We have focused on developing minimally invasive techniques, making this an easier procedure to recover from.

CASA- Sperm Concentration

The concentration of sperm in ejaculation is a major factor in fertility. These number is a criteria for managing male infertility to undergo normal conception, IUI, IVF, ICSI, PESA, TESA etc. The concentration of sperm in semen is automatically calculated by CASA- eliminating human error.

CASA – morphology

In CASA the morphology is determined digitally with utmost accuracy using the special software and avoids cytoplasmic driplets. Abnormal sperm morphology can be an indicator of male infertility. The abnormality can be on head, midpiece and tail.

CASA – DNA fragmentation

The aim of this study is to determine sperm nuclear DNA integrity and mitochondrial function. DNA fragmentation appears to be a cause of male infertility and to be ruled out before IVF/ICSI. Through normal microscopic test we cannot determine this problem 

CASA – Vitality

A method need to differentiate live and dead ones, the non-motile ones may be live which can add to fertility through ICSI. It is analyzed automatically by CASA and cannot be analyzed with manual microscopic test.
